A Case of Myopathy in Osmotic Myelinolysis

Abstract
Osmotic myelinolysis or osmotic demyelination syndrome (ODS) is a demyelinating disease of the pons and other areas of the central nervous system (CNS). The following case report describes a patient who developed critical illness myopathy (CIM) in the setting of ODS. This report presents a 41- year-old male with ODS, whose hyponatremia was corrected. Brain magnetic resonance imaging (MRI) demonstrated the existence of ODS in the central pons and the bilateral basal ganglia. Several days later, muscle enzyme level elevated, electromyography (EMG) and nerve conduction studies demonstrated myopathic findings. The patient showed gradual improvement with conservative management. We report a case of critical illness myopathy, associated with osmotic demyelination syndrome.
